Per le donne al di sopra dei 35 anni di et\u00e0 si sconsiglia tuttavia di \u201caspettare\u201d un anno intero, queste donne dovrebbero sottoporsi almeno ad un esame di base gi\u00e0 dopo 6 mesi.<\/span><\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=&#8221;4.4.0&#8243;][et_pb_column type=&#8221;4_4&#8243; _builder_version=&#8221;4.4.0&#8243;][et_pb_text _builder_version=&#8221;4.4.0&#8243;]<\/p>\n<h2>L\u2019infertilit\u00e0 \u00e8 un problema frequente?<\/h2>\n<p><span style=\"font-size: 16px;\">Il 10-15% delle donne in et\u00e0 fertile riscontra l\u2019impossibilit\u00e0 di concepire, l\u2019infertilit\u00e0 \u00e8 dunque un problema comune della societ\u00e0 odierna e non bisogna vergognarsene.<\/span><\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=&#8221;4.4.0&#8243;][et_pb_column type=&#8221;4_4&#8243; _builder_version=&#8221;4.4.0&#8243;][et_pb_text _builder_version=&#8221;4.4.0&#8243;]<\/p>\n<h2>Di chi \u00e8 la colpa?<\/h2>\n<p><span style=\"font-size: 16px;\">All\u2019incirca nel 40% dei casi la causa risiede nell\u2019uomo e nel 40% dei casi nella donna. Per gli uomini la causa pi\u00f9 frequente \u00e8 una bassa concentrazione di spermatozoi o la loro motilit\u00e0. Per le donne pu\u00f2 trattarsi di una riserva ridotta, o al contrario troppo elevata, di ovuli (la cosiddetta anovulazione) o l\u2019ostruzione delle tube di Falloppio. All\u2019incirca nel 20% delle coppie si tratta di una combinazione delle cause da parte di entrambi i partner. Nel 10% delle coppie non \u00e8 possibile stabilire la causa con i metodi disponibili.<\/span><\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=&#8221;4.4.0&#8243;][et_pb_column type=&#8221;4_4&#8243; _builder_version=&#8221;4.4.0&#8243;][et_pb_text _builder_version=&#8221;4.4.0&#8243;]<\/p>\n<h2>Per quanto tempo possiamo provare prima di richiedere l\u2019aiuto di un medico?<\/h2>\n<p>In genere si consiglia di visitare un centro FIV almeno per una visita di base di entrambi i partner dopo circa un anno di rapporti sessuali non protetti regolari. Con l\u2019avanzare dell\u2019et\u00e0 della donna la possibilit\u00e0 di una gravidanza spontanea e le possibilit\u00e0 di successo di uno qualsiasi dei metodi FIV si riducono, per questo motivo le donne di et\u00e0 superiore ai 35 anni dovrebbero rivolgersi ad un medico prima, si consiglia dopo 6 mesi di sforzi senza successo.<\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=&#8221;4.4.0&#8243;][et_pb_column type=&#8221;4_4&#8243; _builder_version=&#8221;4.4.0&#8243;][et_pb_text _builder_version=&#8221;4.4.0&#8243;]<\/p>\n<h2>\u00c8 possibile essere sotto osservazione da parte di un ginecologo ambulatoriale?<\/h2>\n<p>Dipende nuovamente e principalmente dall\u2019et\u00e0 della donna e dagli esami condotti in precedenza, ad esempio dallo spermiogramma del partner. Il tuo ginecologo ambulatoriale pu\u00f2 tuttavia informarti riguardo alla tua riserva ovarica e puoi seguire mensilmente mediante sonografia la tua ovulazione.<\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=&#8221;4.4.0&#8243;][et_pb_column type=&#8221;4_4&#8243; _builder_version=&#8221;4.4.0&#8243;][et_pb_text _builder_version=&#8221;4.4.0&#8243;]<\/p>\n<h2>Quali esami della fertilit\u00e0 posso aspettarmi?<\/h2>\n<p>L\u2019esame di base per una donna comprende un\u2019ecografia delle ovaie \u2013 per determinare il numero dei cosiddetti follicoli antrali, le dimensioni e la forma dell\u2019utero, l\u2019altezza dell\u00b4endometrio rispetto alla fase del ciclo della donna, eventualmente la prova della perviet\u00e0 tubarica. Viene inoltre eseguito il prelievo del sangue per il cosiddetto profilo ormonale inclusa la determinazione dell\u2019ormone AMH, il quale serve per la valutazione della riserva ovarica nelle ovaie della donna.<\/p>\n<p>Per gli uomini \u00e8 necessario determinare la concentrazione di spermatozoi per un millimetro di liquido seminale, la loro motilit\u00e0 e la loro morfologia.<\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=&#8221;4.4.0&#8243;][et_pb_column type=&#8221;4_4&#8243; _builder_version=&#8221;4.4.0&#8243;][et_pb_text _builder_version=&#8221;4.4.0&#8243;]<\/p>\n<h2>Che cosa significa il termine FIVET?<\/h2>\n<p>La FIVET (Fertilizzazione In Vitro) \u00e8 il metodo di base per la riproduzione assistita. In sostanza si tratta di un metodo di fecondazione dell\u2019ovulo nel quale sull\u2019ovulo vengono versati gli spermatozoi e si attende fino al giorno successivo per vedere se \u00e8 avvenuta la fecondazione dell\u2019ovulo. Questo metodo viene oggi usato in misura minima, solitamente gli ovuli vengono fecondati con il metodo ICSI (intracytoplasmic sperm injection), il quale consiste nell\u2019introduzione di un singolo spermatozoo nel citoplasma dell\u2019ovocito con l\u2019aiuto di un microscopio con ingrandimento elevato mediante un micro ago. Il tasso di successo della fecondazione mediante il metodo ICSI \u00e8 decisamente pi\u00f9 elevato se confrontato con il metodo FIVET, di circa il 70-80%. Il termine FIVET \u00e8 oggi talmente comune, che in genere viene usato durante il ciclo di stimolazione senza che questo metodo venga effettivamente applicato.<\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=&#8221;4.4.0&#8243;][et_pb_column type=&#8221;4_4&#8243; _builder_version=&#8221;4.4.0&#8243;][et_pb_text _builder_version=&#8221;4.4.0&#8243;]<\/p>\n<h2>Chi pu\u00f2 essere sottoposto alle cure in Repubblica Ceca?<\/h2>\n<p>Ai sensi della legislazione ceca pu\u00f2 essere sottoposta a cure nell\u2019ambito della riproduzione assistita soltanto una coppia formata da una donna e da un uomo. Tuttavia ci\u00f2 non dipende dal loro stato civile.<\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=&#8221;4.4.0&#8243;][et_pb_column type=&#8221;4_4&#8243; _builder_version=&#8221;4.4.0&#8243;][et_pb_text _builder_version=&#8221;4.4.0&#8243;]<\/p>\n<h2>Come si svolge il ciclo di stimolazione nell\u2019ambito del ciclo FIVET?<\/h2>\n<p>Durante un ciclo di ovulazione di una donna (circa una volta al mese) in genere si libera un ovulo. Per ottenere un numero maggiore di ovuli \u00e8 necessaria una stimolazione ormonale che dura all\u2019incirca 10 giorni. La donna si applica ormoni stimolanti giornalmente mediante una piccola iniezione sottocutanea. L\u2019applicazione \u00e8 molto semplice e non \u00e8 perch\u00e8 avere paura. Durante la stimolazione ormonale viene tenuta sotto controllo la crescita dei follicoli all\u2019interno delle tue ovaie, dove un follicolo dovrebbe deporre un ovulo. Nel caso di un intervento breve in anestesia totale in genere si riescono ad ottenere all\u2019incirca 8-14 ovuli, tuttavia dipende dall\u2019et\u00e0 e dalla riserva ovarica della donna. Nel giorno del prelievo degli ovuli la donna verr\u00e0 accompagnata nella clinica dal suo partner, il quale consegner\u00e0 gli spermatozoi con i quali verranno facondati i suoi ovuli il giorno stesso. Gli ovuli fecondati, oppure gli embrioni, si sviluppano successivamente in un\u2019incubatrice ed il loro sviluppo viene seguito da un embriologo giornalmente. L\u2019embrione migliore o i due embrioni migliori vengono reintrodotti nell\u2019utero della donna il terzo, quarto o quinto giorno dello sviluppo. Questo intervento \u2013 embriotransfer, \u00e8 solitamente semplice e veloce. I rimanenti embrioni buoni verranno congelati e conservati in azoto liquido. Il tempo di conservazione degli embrioni congelati non incide sulla loro qualit\u00e0 durante un loro successivo utilizzo e possono essere conservati in questo modo per anni. In seguito allo trasferimento dell\u2019embrione la donna dovr\u00e0 prendere per via vaginale dei medicinali per supportare l\u2019annidamento. Dopo due settimane di attesa la donna potr\u00e0 verificare se il determinato ciclo ha avuto successo o meno mediante un test di gravidanza.<\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=&#8221;4.4.0&#8243;][et_pb_column type=&#8221;4_4&#8243; _builder_version=&#8221;4.4.0&#8243;][et_pb_text _builder_version=&#8221;4.4.0&#8243;]<\/p>\n<h2>Cosa fare se la donna non dispone pi\u00f9 di una quantit\u00e0 suffic. di ovuli buoni?<\/h2>\n<p>Soprattutto per le donne di et\u00e0 superiore spesso l\u2019unica possibilit\u00e0 di raggiungere la tanto desiderata gravidanza \u00e8 rappresentata dall\u2019utilizzo di ovuli donati. Questi ovuli sono ottenuti da donne giovani, quindi la loro qualit\u00e0 dovrebbe essere ottimale. As the woman\u2019s age increases her chances of a spontaneous pregnancy as well as chances of success of any IVF method decrease. Therefore, it is recommended for women of 35 years of age or more to seek help sooner, preferably after 6 months of unsuccessful attempts.[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=\"4.4.0\"][et_pb_column _builder_version=\"4.4.0\" type=\"4_4\"][et_pb_text _builder_version=\"4.4.0\" hover_enabled=\"0\"]<\/p><h2>Is it possible to visit a private gynaecologist?<\/h2><p>Again, it is mainly a matter of the woman\u2019s age and the results of the examinations performed so far, e.g. the spermiogram of your partner. Your private gynaecologist can, however, inform you about your ovarian reserve and can observe your ovulation on a monthly basis using sonography.[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=\"4.4.0\"][et_pb_column _builder_version=\"4.4.0\" type=\"4_4\"][et_pb_text _builder_version=\"4.4.0\" hover_enabled=\"0\"]<\/p><h2>What kind of examinations regarding infertility can I expect?<\/h2><p>The basic examination of the woman includes sonography examination of the ovaries \u2013 determining the count of the so-called antral follicles, the size and shape of the uterus, endotermial thickness related to the phase of the woman\u2019s cycle, potentially a test of the patency of Fallopian tubes. There is also a blood sample taken in order to ascertain the so-called hormonal profile including the determination of the AMH hormone, which is used to assess the egg supply in the woman\u2019s ovaries.<br \/>The man has to undergo the determination of the sperm concentration in one millilitre of the ejaculate, their agility and morphology.[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=\"4.4.0\"][et_pb_column _builder_version=\"4.4.0\" type=\"4_4\"][et_pb_text _builder_version=\"4.4.0\" hover_enabled=\"0\"]<\/p><h2>How should I understand the term \u201cIVF\u201d?<\/h2><p>IVF (In Vitro Fertilisation) is the basic method of assisted reproduction. It is in essence a method of fertilising the egg during which sperm is poured over an egg in a dish and then left for one day to fertilise the egg. This method is used today only in marginal amount of cases, the eggs are most commonly fertilised using the ICSI (IntraCytoplasmic Sperm Injection) method, which is based on introducing a single sperm into the cytoplasm of the oocyte in a microscope under strong magnification using a microneedle. The success rate of the fertilisation using ICSI method is many times higher compared to IVF, namely 70-80%. IVF is, however, a term so common that it is used in connection when talking about cycle stimulation in general, despite the fact that the IVF method is rarely used nowadays.[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=\"4.4.0\"][et_pb_column _builder_version=\"4.4.0\" type=\"4_4\"][et_pb_text _builder_version=\"4.4.0\" hover_enabled=\"0\"]<\/p><h2>Who can be the receiver of the treatment in the Czech Republic?<\/h2><p>The treatment in the area of assisted reproduction may only be administered to a couple of a man and a woman according to Czech legislation. However, they do not have to be married.[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=\"4.4.0\"][et_pb_column _builder_version=\"4.4.0\" type=\"4_4\"][et_pb_text _builder_version=\"4.4.0\" hover_enabled=\"0\"]<\/p><h2>How is the stimulation cycle performed within the IVF cycle?<\/h2><p>The usual situation is that during one ovulation cycle (approx.once per month) one egg is released. To obtain a larger quantity of eggs hormonal stimulation is necessary, lasting usually for about 10 days. The woman gives herself stimulating hormones using a small intradermal injection on a daily basis. The injection of the hormones is very easy and there is no need to fear it. During the hormonal stimulation the growth of the follicles in your ovaries is checked. One follicle typically contains one egg. After a short operation performed in general anaesthesia usually between 8 and 14 eggs are obtained, the specific number depending on the age and ovarian reserve of the woman. On the day of the operation you will be accompanied to the clinic by your partner who will submit his sperm, which shall be used on the same day to fertilise your eggs collected. The fertilised eggs, or embryos, undergo further growth in an incubator and their development is observed by an embryologist. One or two best embryos are introduced back into the woman\u2019s uterus between the third and fifth day after the operation. This operation \u2013 embryotransfer \u2013 is usually easy and quick. The rest of the embryos will be frozen and stored in liquid nitrogen. The length of the storage has no impact on their quality during their further usage and the embryos may be stored for many years. After the embryotransfer you will use medication to support the nesting of the egg, which shall be applied intravaginally. Two weeks later you will learn from the pregnancy test whether the given cycle was successful or not.[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=\"4.4.0\"][et_pb_column _builder_version=\"4.4.0\" type=\"4_4\"][et_pb_text _builder_version=\"4.4.0\" hover_enabled=\"0\"]<\/p><h2>What is the success rate of the IVF treatments?<\/h2><p>In general the IVF therapy success rate is stated between 40-60%. It depends on many factors and circumstances, the most prominent of which is undoubtedly the woman\u2019s age. Younger women, aged 37 and less, have a larger chance to get pregnant, namely around 60%. On the other hand the chances of a woman aged 42 and more are 5-10%. It is therefore clear that the sooner, the better.[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=\"4.4.0\"][et_pb_column _builder_version=\"4.4.0\" type=\"4_4\"][et_pb_text _builder_version=\"4.4.0\" hover_enabled=\"0\"]<\/p><h2>What to do when the woman does not have a sufficient amount of quality eggs?<\/h2><p>Especially in older women usage of donated eggs is often the only chance to achieve the desired pregnancy. These eggs are obtained from young women, so their quality should therefore be optimal.
